Registration

Waiver and Release of Liability

By submitting the online payment and/or registration form(s), the participant (or the participants guardian), gives consent to participate in this Nona Sports, LLC sponsored program.  I hereby release, discharge, and otherwise indemnify Nona Sports LLC, Nona Lit Inc., City of Orlando, Orange County Florida, the State of Florida, directors, managers, employees, contractors, agents, coaches, assistant coaches, team parents, volunteers and officials, against any claim by or on behalf of the participant.  I take full responsibility for any injuries incurred or resultant of death.  In the case of emergency, call the emergency contact(s) provided.  At the discretion of Nona Sports LLC, care may be given under whatever conditions are necessary to preserve the life, limb, or well being of the participant.

I understand that insurance will not be furnished by Nona Sports LLC, City of Orlando, Orange County Florida nor the State of Florida.

By submitting the online payment and/or registration form(s), the participant (or the participants guardian), acknowledges that volleyball or any sporting event is an extreme test of a person's physical and mental limits and that participation in a volleyball event can cause death, serious injury, or property damage.  With a full understanding of the potential risks, I hereby understand the risks of participating in volleyball or any sporting event.

I, the participant (or the participants guardian), acknowledge that additional information may be received regarding additional Nona Sports LLC sponsored programs.

During 2012 the Florida Legislature passed House Bill 291 to insure that parents, coaches and athletes are informed of the risks associated with head injuries and to mandate minimum standards for education, informed consent and re-entry after suspected injury. 

The law requires the following:

1. Education of athletic coaches, officials, administrators, and youth athletes and their parents or guardians of the nature and risk of concussion and head injury.

2. The parent or guardian of a youth who is participating in athletic competition or who is a candidate for an athletic team to give consent each year before participating in athletic competition or engaging in any practice, tryout, workout, or other physical activity associated with the youth’s candidacy for an athletic team.

3. Each youth athlete who has been removed from an activity due to injury may not return to practice or competition until the youth submits to the director and/or coach a written medical clearance to return stating that the youth athlete no longer exhibits signs, symptoms, or behaviors consistent with a concussion or other head injury.

4. Medical clearance must be authorized by the appropriate health care practitioner trained in the diagnosis, evaluation, and management of concussions as defined by the Sports Medicine Advisory Committee of the Florida High School Athletic Association.

Covid-19 Release of Liability

The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Nona Sports LLC has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that the player, player's guardian, their family and/or guests will not become infected with COVID-19. Further, attending any Nona Sports LLC program or activity could increase the risk of contracting COVID-19.

 

By submitting the online payment and/or registration form(s), the participant (or the participants guardian), acknowledges the contagious nature of COVID-19 and voluntarily assume the risk that the participant and I may be exposed to or infected by COVID-19 while attending and/or participating in the Nona Sports LLC program or activity and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Nona Sports LLC program or activity may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Nona Sports employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, permanent disability or death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the Nona Sports LLC program or activity. 

 

By submitting the online payment and/or registration form(s), the participant (or the participants guardian), hereby release, covenant not to sue, discharge, and hold harmless Nona Sports, City of Orlando, Orange County and the State of Florida, its volunteers, employees, agents, and representatives, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Nona Sports , its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending any Nona Sports program or activity.

Pictures and Videos Release Acknowledgement

I, the participant (or the participants guardian), by submitting the online payment and/or forms give consent to Nona Sports LLC to use pictures and /or videos taken of the participant for use in educational review, social media and/or promotional purposes.  I also acknowledge that, due to privacy issues, I will refrain from posting pictures and/or videos on group chats and/or any social media platform (i.e - Facebook, Instagram, Twitter, Linkedin, TikTok, Pinterest, etc.) without prior consent.
